Description
This program will start by introducing the steps and components to establish collaborative practice agreement in any settings and will feature best practice models of developing and implementing unique collaborative practices and medication management programs in various community pharmacy settings (independent, academic medical center, chain, etc). Practitioners will share the challenges, successes and opportunities in developing patient care programs in their pharmacy setting.

Learning Objectives
At the completion of this activity, the participant will be able to:

  1. Discuss pharmacist scope of practice regarding collaborative practice agreements (CPAs) and patient care programs in the community pharmacy setting.
  2. Explain the process and components of collaborative practice agreement.
  3. Compare and contrast patient care programs developed in different community pharmacy practice sites.
  4. Discuss various challenges in developing collaborative practice agreements and patient care programs and strategies to overcome those challenges.

Continuing Pharmacy Education
The California Pharmacists Association (#0113) is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. Upon completing an evaluation form, participants will be awarded 1.25 hours (0.125 CEUs) of Continuing Pharmacy Education Credit. A statement of credit will not be issued.
